How do I apply for Head Start or Early Head Start?
To apply for Head Start or Early Head Start you should:

Call your local program. To find a program in your community, you may use the national Head Start Program Search Tool or call the Head Start Information and Publication Center toll-free at 1-866-763-6481. You may apply over the phone. You will need to provide some information about your family and your child such as:
Child’s name
Child’s date of birth
Address
Telephone number
Parents' names
Family income

Shortly after the phone intake, someone will call you to set up an appointment to come to your home. During the home visit, you will need to fill out an application and may be asked to provide the following information (as applicable to your situation):
Birth Certificate or Baptismal Record
Immunization Record
Class schedule showing parent's school status or job training
Certificate of Indian Blood
Child's Social Security Card
Current Medicaid card or other insurance
If your child has a disability, copies of child's IEP/IFSP
Children in protective custody or living with foster parents, documentation of this placement will be needed
All the information will be sent to a social worker at the Head Start / Early Head Start program your child will attend. You will be contacted as soon as there is an opening.
